People with diabetes are reviewed regularly by the Practice Nurse. If you have not been checked within the last 12 months please make an appointment with the Practice Nurse on 01506 655155.
The CPNs deal mainly with anxiety and depression. A referral is by one of the doctors. The CPN can be contacted via the Community Reception desk on 01506 630771.
Referrals are made by a doctor. Follow up will be arranged through the clinic and drug supervision clinics.
Your child will be invited to attend for Diphtheria, Tetanus, Pertussis, Meningitis C, Polio, HIB and MMR at appropriate ages. These clinics are run by the Health Visitor on Tuesday mornings.
Women aged 25 – 64 years of age are advised to have 5 yearly checks. Please make an appointment with the Practice Nurse.
