Pharmacy team
Health board pharmacists and pharmacy technicians are becoming more involved in the care of patients in General Practice. Having the pharmacy team working in GP surgeries provides valuable medicines support, a close link with community pharmacy teams and allows GPs to focus their skills and time where they are most needed for patients who have more complex needs. Pharmacists are experts in medicines and can talk to you about how to get the best from your medicines.
Pharmacy Hub team
To further support GP practices, a new service is being launched to implement a remote pharmacy hub, which will be based within Stoneyburn Health Centre.
This service will predominately deal with Hospital Discharge letters and again be staffed by a central pharmacy team with advanced clinical skills.
Our hub team may contact patients to discuss medication changes relating to recent hospital discharges. As this team are based out with the practice at Stoneyburn Health Centre, the telephone number used to contact patients will differ from the practice telephone number.
